The City of Sioux Falls Health Department is accepting applications for a part-time Nurse Case Manager at Falls Community Health clinic.

At Falls Community Health (FCH) it is our mission is to be an open door to quality health care services by removing barriers, especially financial, that exist for working families without health insurance or other resources to meet their health care needs.

Falls Community Health is a community medical and dental clinic for all ages. The goal of FCH is to reduce health inequalities by providing high quality, accessible, and affordable health care. FCH serves patients at four sites: the main site and three school-based sites, and all services are available to residents regardless of their ability to pay.  Services include:

  • Team-based primary care.
  • Evidence-based care for acute and chronic illnesses.
  • Well-child care
  • Immunizations
  • Laboratory and X-ray services.
  • HIV/AIDS early interventions and case management
  • Behavioral health services.
  • Support for self-management of your health.
  • Case management and social services.
  • Family planning


The nurse case manager engages with patients who are challenged with complex health issues or who have a lack of economic, social, and interpersonal resources. In addition, the nurse case manager facilitates a plan of care along with the patient to achieve health and quality of life goals. This position promotes education, self-management support, and timely healthcare delivery. Nurse case management is part of, and coordinates care with, the clinic's care teams, which are interdisciplinary teams of clinical and non-clinical staff. Duties may also include accompanying patients to medical appointments outside of FCH, care planning and networking with local and state agencies.

Qualified applicants must possess computer skills (preferably with an electronic health record), excellent communication skills, and a professional attitude. The qualified applicant must hold a State of South Dakota Board of Nursing Certificate as a licensed registered nurse. Previous case management experience with refugee health is strongly preferable.


This is a non-benefitted position working up to 28 hours per week, Mon-Fri, based on clinic needs.
